We’re in-network with Regence BlueCross BlueShield, Premera, Providence, Aetna, Cigna, UnitedHealthcare, Kaiser Permanente, and Medicare. Self-pay? Prices are posted in the lobby and on every estimate — physicals are $180, most labs start at $12.

Yes — follow-ups, medication checks, behavioral health, and minor illness reviews all work well over our secure video platform. We do recommend your first-ever visit be in person so we can establish care properly.

Request through the patient portal before 2 PM and it’s sent to your pharmacy the same day. Controlled substances require an in-person visit, in line with Oregon law — we’ll book you quickly when one is due.

Free 90-minute validation in the Raleigh Street garage directly behind the clinic. On transit, the Portland Streetcar NS Line stops at NW 23rd & Marshall — two blocks north — and TriMet bus 24 stops on our corner.

Photo ID, insurance card, a list of current medications, and any vaccination records you have. Complete the intake forms in your portal beforehand, or arrive ten minutes early and we’ll do it together at the front desk.
